The PCN Community Occupational Therapist service
An occupational therapist helps people of all ages who have permanent or short-term physical, sensory, mental, or communication challenges. They work to improve their independence, allowing them to take part in activities that matter to them. This is done by identifying and removing barriers in their daily life. Sometimes, even a short intervention from an occupational therapist can prevent hospital admission or long-term injury.
How can our Occupational Therapy Service help you?
OT’s, short for Occupational Therapists work with people to improve participation and independence in daily activities. We are experts at analysing activity, the environment and occupations and work alongside people to problem solve difficulties that individuals may be having because of illness, injury or through getting older. Occupational Therapists ask ‘what matters to you?’ not ‘what’s the matter with you?’
We can work with what is meaningful to you. If it matters to you, it matters to us! Our aim is to keep you safe, functional and well within your home and community.
What do we cover?
- Mobility – walking inside and outside, standing up from sofas/chairs, managing stairs, getting in/out of bed
- Accessing the community – step access, ramps, rails and getting in/out of the car.
- Personal care – advice and equipment for toilet transfers, getting in/out the shower or bath, techniques and strategies to remain independent.
- Equipment – advice and provision, wheelchair referrals, walking aids, specialist chairs.
- Domestic and household – preparing food/drink, budgeting, household chores.
- Environment – advice and adjustments to make your home safer and reduce falls risk
Who can access the Occupational Therapy Service?
The service supports adults aged 18 and over across the Cerne Abbas, Milton Abbas, and Puddletown Surgeries, helping make occupational therapy more accessible to individuals in our rural communities.
We work with people living with long-term conditions, supporting goal setting, promoting independence, and offering care for those with palliative needs or approaching end of life.
While we do not provide urgent or rapid response care, and do not deliver physiotherapy rehabilitation (this is managed by the Integrated Community Rehabilitation Teams – ICRT), we work closely with them to ensure joined-up, holistic support.
