About Us

What is a PCN?

 Since it was established in 1948, the NHS has witnessed significant changes, including a growing population and longer life expectancies. With these changes, more people are managing long-term conditions such as diabetes, heart disease, and mental health issues, leading to increased demand for local health services.

To meet these needs, GP practices are now working together with community health services, mental health services, social care, pharmacies, hospitals, and voluntary organisations in their local areas. These collaborations are organised into groups known as Primary Care Networks (PCNs).

Mid Dorset PCN

The Mid Dorset Primary Care Network, established in 2019, consists of nine GP practices serving the Mid Dorset area, collectively catering to over 50,000 patients.

While each practice maintains its independence, our collaborative approach unlocks numerous opportunities. By working together, we can expand the scope of services available to patients and seamlessly integrate with the broader health and social care system. This co-operation ensures that patients receive comprehensive, co-ordinated care tailored to their needs.
