Ageing Well Clinic (Starting 14th July 2026)

Who we are and what we do:

The Ageing Well team is a group of specialised healthcare professionals with expertise in ageing and the challenges that can come with getting older. We offer a comprehensive, “one‑stop‑shop” style clinic designed to support you with personalised advice, guidance, and treatment plans that promote your health, wellbeing, mobility, and independence.

Our aim is to identify concerns early, understand what matters most to you, and work with you to create practical strategies that help you age well. By doing this, we hope to empower you to remain as independent as possible and enhance your wellbeing.

What will happen at the clinic?

The Ageing Well Clinic appointment last approximately two hours. Toilets and light refreshments will be available if needed. You are welcome to bring a relative or friend with you if you feel this would be helpful.

During the appointment, you will meet several members of the Ageing Well Team, including:

 

General Practice Assistant / Health Care Assistant

They will carry out your clinical observations, take blood samples if required, and review the information provided in your pre‑appointment questionnaire.

Referral Process:

You may be referred to the Ageing Well Clinic by your GP or another healthcare professional if they feel you are showing signs of “pre‑frailty”.

These signs may include:

·         You feel you have “slowed down”.

·         You are beginning to experience changes in your mobility (feeling unsteady, requiring more assistance, fear of falling).

·         You have experienced one or more falls in the past 12 months.

·         You are feeling less confident / less able with domestic or physical tasks (including washing & dressing, housework, shopping).

·         You are starting to feel more isolated socially.

·         You are feeling overwhelmed by a long list of medications.

If you feel you are experiencing early changes yourself and would benefit from a holistic assessment, you can also self‑refer.

If you are a carer, or you support someone who may benefit from a free health and wellbeing check, or support someone you feel may be experiencing any of the criteria above, please contact the Ageing Well Team to discuss further.

Advanced Care Planning:

Advance Care Planning (ACP) is a core component of an Ageing Well Clinic, supporting proactive, person‑centred care for older adults. It enables people to express their values, priorities, and treatment preferences before a health crisis occurs. This helps ensure that future care aligns with what matters most to you and reduces unnecessary or unwanted interventions.

ACP provides clinical teams with clarity on a person’s preferred place of care, views on hospital admission, and decisions regarding treatment escalation, including resuscitation and end of life care. For people living with long‑term conditions, having these plans in place helps to share your decisions and communicate your wishes which may become more challenging as your condition progresses.

From a service perspective, a shared ACP helps to improve communication, making sure everyone involved from families and friends to healthcare professionals are aware of your decisions, ensuring people receive treatment that reflects their wishes and promotes the best possible quality of life as their needs evolve.

During your Ageing Well Clinic appointment, these topics will be discussed in more detail to help ensure your care reflects your wishes. It may be helpful to spend some time thinking about what matters most to you — including your preferences for future treatment, hospital admission, and place of care — before attending the appointment. This can support a more meaningful and personalised conversation.
