PCN Mental Health Practitioner

A Mental Health Practitioner supports people with complex mental health needs and acts as a 'bridge' between primary care and specialist mental health services. 

How can our Mental Health Practitioner help you?

Our Mental Health Practitioner will work with you too:

  • Support shared decision making about self—management.
  • Facilitate onward access to treatment services.
  • If deemed appropriate following assessment by MH Practitioner, an individual could be offered brief psychological interventions (up to 6 sessions) that are not duplicated elsewhere in the system, i.e. CMHT, Steps2Wellbeing and third sector.
  • Work alongside the MDT to address a range of biopsychological needs of people with mental health problems.

Who can access the Mental Health Practitioner service?

Our Mental Health Practitioner, Becci, is based at Maiden Castle and can see patients from all our surgeries by either face-to-face or telephone consultations. 

Referrals are ONLY through your GP, so contact your surgery first to see if you are eligble for a refferal.
