Privacy Statement

There are many people on our community who care, unpaid, for family and friends. Looking after someone else can be difficult and if we are able to identify our patients who are caring for others, it will help us to support you. If you are an unpaid carer, please let us know by completing the form below.

A code will be added to your medical record to identify you as an unpaid carer so practice staff will be aware, your name will also be added to the carers register at your practice so your surgery may support you with the following:

·         all newly registered carers will receive a call or letter from a carers lead and a carers information pack

·         further support to take care of your own health, such as free flu vaccinations

·         with your consent, make referrals to dedicated carers services for support

·         carers clinics, in partnership with Carers Support Dorset, enabling carers to meet privately with a carer adviser for personalised advice and support (offer varies between surgeries)

·         with consent obtained, information on any medical conditions and treatments for the person you care for, to help you fell more confident caring

·         repeat prescriptions sent to your local pharmacy, to save you picking them up.

If your situation changes and you are no longer a carer, please do let us know and we will remove you from the carers register and update your health record accordingly. Further information about how we use and process your information can be found on our website.
